An open-label, phase I trial was conducted to evaluate acute respiratory and sedation safety in children adolescents between the ages of 12 and 18 years who self-medicated with heroin. The trial aimed to establish doses of buprenorphine sufficient to produce adequate analgesia, sedation and with analgesia as a secondary outcome and to minimize unwanted respiratory effects. The trial was conducted from January 2009 through October 2010 in four clinics, two of which were primary care-based. Twelve participants attended all four clinics and completed a daily active treatment protocol comprising oral, sublingual, subcutaneous or buprenorphine inhalation sublingual absorption of a single dose 100 mg of naloxone sublingually administered. One participant was excluded from analysis due to self-reported injection drug use. In total, 13 participants enrolled this trial and completed the protocol. participants' demographic information, medical history, substance abuse histories, drug assessment measures, opioid dependence measures and use abuse patterns were recorded at baseline and every 28 days. RESULTS: The safety of buprenorphine in children on a daily buprenorphine treatment protocol including 100 mg of naloxone was comparable to that in the current clinical practice. Adherence to the protocol was high and self-reported drug abuse was low. The participants were predominantly Caucasian, aged between 10 and 17 years, had a median follow-up period of 1.7 months and had a median starting opioid dose of 10.7 mg. Participants had a median daily dose of 12.8 mg intravenous heroin at baseline, were mostly non-Hispanic White, had a median daily opioid dose of 40.13 mg heroin at baseline and had a mean follow-up opioid dosage of 43.4 mg. In this study, no opioid-related adverse events occurred. However, when evaluated overall after 28 days the last opioid dose, 19 participants (6.4%) discontinued buprenorphine as a result of adverse effects, the majority which were related to respiratory toxicity. The rate of opioid-related respiratory toxicity was substantially different when compared to baseline and after each of the 3 weeks follow-up evaluation periods (P = 0.004). CONCLUSION: The study demonstrated that current buprenorphine treatment protocol for opioid abuse in children and adolescents does not produce any significant drug-related respiratory toxicity while providing significant pain management and analgesia. These findings support the rationale for extended duration drug consumption when combined with opioid overdose prevention (such as when given in combination with naloxone) as recommended by USPSTF.
